What is necessary?
You must bring the following:

A GOVERNMENT ISSUED PHOTO ID

Documentation of your childhood immunizations:

MMR (measles, mumps, rubella)

Varicella (chickenpox) or history of disease

a tetanus shot in the last 10 years

one Covid vaccination

a Flu shot if between October and April

IF YOU DO NOT HAVE THIS DOCUMENTATION, YOU WILL NEED TO OBTAIN THESE IMMUNIZATIONS AT PUBLIC HEALTH
CONCIERGE MEDICINE
Dr. Blue provides 24/7 concierge medicine to individuals and families by arrangement. He also provides travel consultations and has, on special request, provided accompaniment with clients who want emergent medical services available during remote travel.
